Who Qualifies?

If you are at risk for diabetes, most insurances** will cover the program at no cost to you. Participants must meet the following criteria:

  • Age 18 or older and have a body mass index greater than or equal to 24 (22, if Asian); and ƒ

  • A diagnosis of prediabetes using a blood test within the past year or a history of gestational diabetes mellitus (GDM) or

  • A score on the CDC Prediabetes Screening Test that shows high risk for prediabetes.
